I find that most if not all of the time I hit that shot my left (lead) hip is pointing to the extreme left. I guess that I'm opening my hips too early and not facing the ball at impact which has been a perpetual problem. I also think that I'm picking the club up too early in my backswing and chopping down on the ball. I have a dent on the top of my 5 Wood as proof. I've got to admit that I'm not doing the PVC drill as much as I should. Any tips/drills will be greatly appreciated.

You must get the club path from the inside in the downswing. I suggest working on hitting easy shots until you can feel the club move from the inside out. Work on position 3 with the pvc drill. Todd
